Cognitive effects are the least supported
Across trials in young adults, anxiety and physiological stress show small-to-moderate effects while cognitive performance is the most variable outcome measured.

- Ten interventional trials published between 2015 and 2025: eight randomised controlled trials, two crossover studies, and two quasi-experimental pre-post designs, together covering approximately 970 participants with individual sample sizes from 18 to 262. Participants were predominantly college and university students aged 19 to 24, with a smaller number drawn from perioperative and dental settings.
- Result
- Anxiety reduction showed small-to-moderate effects, with standardised differences from 0.36 to 0.47, and larger effects in clinical anxiety contexts than in student samples. Theta-frequency binaural beats showed a stronger effect on physiological stress markers, around 0.59, alongside increases in high-frequency heart-rate variability. Cognitive performance was the most variable domain: where an effect size could be extracted at all it was around 0.34, and in part of the literature it could not be extracted. Beta-frequency beats were associated with more modest cognitive benefits. The reviewers concluded that these interventions can beneficially modulate sleep, anxiety, and cognitive processes, while stating that heterogeneity and small samples limit the certainty of that conclusion.

Risk of bias across domains was 52 per cent low, 44 per cent some concerns, and 4 per cent high; recent pre-registered trials scored well while earlier ones reported randomisation and attrition incompletely. Heterogeneity in intervention parameters was substantial enough that the reviewers judged a definitive meta-analysis impossible. Some effect sizes were read off figures or estimated from reported data rather than taken from stated values, which introduces imprecision. The reviewers state that publication bias cannot be ruled out because trials reporting null results may be underrepresented.
Limitations

- Publication bias cannot be ruled out; trials finding nothing may be underrepresented.
- Many included studies were early-phase, with small samples and limited blinding.
- The trials tested binaural-beat audio in general. None of them tested audio generated by Fluctara.
